We understand that the laws which protect the confidentiality of medical information apply to telepsychiatry. Your video telepsychiatry visit with Dr. Baez private practice will not be recorded and all identifying information in the interaction will be kept secure in the same manner as any other private medical information in compliance with the HIPAA-compliant security and end-to-end encryption of medical records and images.
Our system does log your IP address (your computer’s Internet address) to track visitors and to administer the site. However, IP addresses are not linked to any personally identifiable data. The only other information automatically supplied to us is the information provided by your browsers, such as the type of computer and operating system you are using. The only personally identifiable data we collect is provided to us by the visitor, either via e-mail, online information request, survey, or site registration.
We may also request non-personally identifiable information, though this information is optional. We may use this data to improve our site and/or tailor your experience at our site, showing you content that we think you might be interested in and display content according to your preferences.
All patient information is confidential and cannot be released to a third party except with specific authorization by the patient. However, there do exist few limits to this confidentiality:
- If a child or elder abuse is revealed in the context of a therapeutic relationship, this must be reported to the proper authorities, and confidentiality may be broken.
- If the patient poses a serious danger to himself/herself or others, confidentiality may be broken in an attempt to provide medical/mental health treatment and/or safety.
- I am obligated to warn a person if I think this person is likely to become a victim of violence at the hands of a patient.
Please keep in mind that communications via email over the internet are not secure despite us using a HIPAA compliance email account. Although it is unlikely, there is a possibility that information you include in an email can be intercepted and read by other parties besides the person to whom it is addressed. Please do not email us any clinical information and instead use the patient portal for secure communication of clinical information.
We will not sell, share, or rent the information to others. This information will not be shared with other organizations.

Upon your request, we will provide you with a summary of the information we collect about you. You will have an opportunity to correct, update or modify this information.
