  • What do I do if I feel like harming myself or others?

    Contact 911 and provide details of your current location, notify any family, friends or others that are in your proximity and or contact the National Suicide Prevention Hotline at 1-800-273-8255.

  • Where do I go if I have a medical emergency or adverse reaction to a medication?

    Go to the closest emergency room if you are capable of driving otherwise call 911 and then a relative. Please make sure to take all prescribed medications if visiting the emergency room and ask the staff to contact your doctor as soon as possible.

  • How do I get help or help my loved one?

    The first step for you or your loved one to get help is to recognize that there is a problem.

    If your life or the life of others is in danger or you feel in so much pain that it is imminent that you want to harm yourself or others please call 911 or 211.

    Otherwise, complete the Appointment Request Form and the office staff will contact you to discuss your options for an evaluation to determine yours, his or her psychological needs.

  • How do I access the patient portal?

    The patient portal is a HIPPA-compliant medical platform where you can communicate safely with Dr. Baez and the practice staff.

    The Patient Portal enables the patient to log in online and fill out all required forms: consent for treatment, disclosures of information, HIPAA privacy notices, demographic information forms, clinical questionnaires, etc.

    Once you are scheduled for your initial visit with Dr. Baez you will receive an email from the staff during working hours or during the weekend if your appointment is scheduled for a Monday with sign-in and document completion instructions.

  • How do I request a prescription refill?

    You will need to log in to our secure Patient Portal of Luminello e-Records and send a message to the support staff indicating the name of the medications (s) that you are in need. If your physician has failed to send your prescription or has made a mistake in the number of refills your prescription will be filled until your next recommended appointment.

    If you are due to be seen the practice will not be able to send a refill to your pharmacy and you will need to Schedule a Follow Up Session Now in the first available appointment. Please see our Prescription Refill Policy for additional details.
